Patient Participation Group

Patient Group

At Swallowfield Medical Practice we are always looking to improve the services we provide to patients and your views really do matter to us. To provide a patient perspective when we look to develop healthcare and offer new and enhance existing services we formed a Patient Participation Group.

The Group consists of our Senior GP Partner, The HR/Administration Manager and nine volunteer representatives from amongst our patients. The Practice Manager and other staff and external colleagues both from within healthcare and the wider statutory and voluntary services also attend to discuss specific issues. The group meets quarterly to discuss a variety of issues regarding Swallowfield Medical Practice and the services provided to its patients.

We look forward to hearing any ideas and opinions you might have. Please contact the practice or speak to any of our members below or email them at ppg.smp@nhs.net:

Virtual Group

To enable wider patient participation, we also have a Virtual Patient Reference Group. They would like to be able to ask the opinions of as many patients as possible and are asking if people would like to provide their email addresses so that they can contact you by email every now and again.

Your contact details will only be used for this purpose and will be kept safely.

If you are happy for us to contact you occasionally by email please complete and submit our registration form.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance
