Here at Swallowfield Medical Practice we thought that our Patients might appreciate a Text reminder on the mobile phone of their coming appointment. This reminder will advise you on the Time, Date, Clinician and Location of your appointment so that you can schedule the rest of your busy day around it.  Please note that no charge will be incurred by you for this service.

One of the benefits for using this reminder system is that it will help us to reduce the number of missed appointments which are a terrible waste of resources. By cutting down on these missed appointments  we will be able to offer a wider choice of appointments to those people who need to be seen at short notice.
In order to provide this service to you, we will need to have your mobile phone number on our records. At the same time if you do not wish to be reminded of your future appointments you can request it by filling and submitting the form below. 

We would like to encourage all our Patients to register their number. For parents, please register your own number for your children so we can advise you of any Immunisation campaign as well as their appointments. Once your child reaches 16 years of age we would advise them to register their own mobile number.
