| When you register with a GP Practice you
are required to fill a Registration Form, a New Patient Questionnaire and you
will also receive some important lifestyle advice. These forms and the leaflet are available for collection at our Swallowfield Surgery reception but for your convenience we have made them available on line. Please fill in All the boxes to avoid delays in your registration process. By printing the form you will also print the lifestyle advice leaflet and the welcome letter which has some important instructions. Giving the lifestyle advice leaflet is a requirement for all new registrations and this will be entered as an event in your medical record.
