Carers

Do you look after someone who is ill, frail, has learning or physical disabilities or is mentally ill?

If so, you are a Carer.  It is estimated that there are approximately 6 million carers in Great Britain with 1 in 5 households containing a carer.  Caring for someone is a valuable but stressful role and all carers, regardless of age, will benefit additional support whether it be in the form of additional help, information or just someone to talk to. 

At Swallowfield we have a Carer's register which identifies those patients who are caring for others.  enabling our GPs to better understand the health and social needs of the Carer.  There are many other  organisations who are able to offer help and support, a list of some of these can be accessed here

If you would like to be added to our Carer's Register please could you complete and print  the Carers Identification Form and return it at your earliest convenience.
