Vascular Assessment Programme

Swallowfield Medical Practice is taking part in a new disease prevention programme, in partnership with NHS Berkshire West.

Heart disease and Stroke are the UK’s biggest killers, but they can be prevented through early detection of risk factors and treatment.  We have identified and will be contacting patients of ours who we think would most benefit from a vascular assessment. The check will be carried out by our Health Care Assistant, Tracie Johns. 

 The assessment includes: 

• Blood pressure measurement
• Height, weight and waist measurement
• A lifestyle assessment, which will help you to understand how your diet and activity levels could affect your health.
• Gathering information about your family medical history of heart disease or stroke
• A simple blood test to check your cholesterol and blood sugar levels.
